These Terms govern your use of the Elevate Optimal website, eligibility assessment, checkout, support channels and related services, operated by Elevate Optimal Inc. ("Elevate Optimal"), 7757 N Artesiano Rd, Tucson, AZ 85743. By using the website you agree to these Terms and the policies linked from them.

Elevate Optimal is a technology platform, not a medical provider, and it is not a pharmacy. Medical services are provided by LocumTele, whose licensed nurse practitioners and physician assistants review assessments and write prescriptions. LocumTele is responsible for clinical care, provider licensure and oversight, patient records and clinical policies. Elevate Optimal Inc. provides the website, your account and non-clinical support, and cannot influence a clinical outcome. A provider-patient relationship begins only when a licensed provider reviews your information and establishes it. Website content, the assessment, an automated message or a support conversation does not by itself create a provider-patient relationship or guarantee treatment.

Elevate Optimal is not an emergency service. Call 911 or go to the nearest emergency room for an emergency.

You must be 18 or older and located in a state we serve (see State availability). You agree to give accurate, current and complete information and to update it when it changes.

A licensed provider decides whether an evaluation, treatment, prescription or follow-up is appropriate. We do not guarantee eligibility, a prescription, a specific product, a particular result or a specific review time.

Prescriptions are filled after a licensed provider issues a valid prescription. Compounded medications are prepared and dispensed by VSDH, a state-licensed compounding pharmacy; FDA-approved medications are dispensed by a state-licensed pharmacy. Compounded medications are not FDA-approved, and the FDA does not review them for safety, effectiveness or quality. Follow the pharmacy label and your provider's instructions. Never share prescription medication.

You're only charged if a licensed provider reviews your assessment and approves treatment, usually within 24–48 hours. If treatment isn't approved, you aren't charged and nothing ships. Plans are billed monthly and renew automatically at the same price every month until you cancel. You may not use the website unlawfully, submit false information, access another person's account, interfere with the service or copy content without permission.

These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-law rules.
